Appointments

The Northern Neighbours NPLC will strive to offer urgent same day appointments to patients. The aim of the clinic is to have patients call when they are unwell and have them seen by the Nurse Practitioner in a timely manner. This will provide you access to your health care provider when you really need it.

When you make an appointment please tell the receptionist your reason for the visit, this allows for the appropriate amount of time to be booked for your appointment.

If you are going to miss an appointment, please provide at least 24 hours notice or a fee may apply. A late cancellation policy is in effect.

Prescription Renewal

In order to properly assess your medication needs, prescription renewals may require an appointment with your primary health care provider.

Please note: It may take between 2-7 days for your renewal to be completed.

Lab Results

If you have an abnormal lab test, you will receive a call from us. We do not call you if the results are normal. Patients can also obtain their lab results by contacting one of the NNNPLC Nursing Staff (RN/RPN).

Additionally, you may wish to check your results with the Nurse Practitioner at your next visit to the Northern Neighbours Nurse Practitioner-Led Clinic.

Notes, Letters and Form Completion

If you require a form to be completed for either school, work or insurance, you will be required to book an appointment with your primary health care provider. Please note that fees may apply for documentation.

Clinic Arrival

Please check in with reception and bring your health card to each visit. Inform reception of any changes of name, address, work or phone numbers.
We try to be on time and we would like you to be also. You may have a short wait if we have an emergency or other complication. If you are late for your appointment, you may be asked to re-book for another time.

Patient Forms

Patient Registration Form

To become a patient or client of the Northern Neighbours Nurse Practitioner-Led Clinic, you may register directly by clicking Patient Registration Tab. The completed form will then be submitted directly to the clinic. Once your registration is received, you will be contacted by our receptionist for an appointment.

Alternatively, you may register in person at the clinic. Please see receptionist to request the patient registration form or for more information.

Professional Referral Form

This form is used by allied professionals and/or service providers when referring patients or clients to Northern Neighbours Nurse Practitioner-Led Clinic for services.

Patient Survey

This survey form is for existing patients that would like to submit feedback about the clinic and their experience. We encourage everyone to provide any and all feedback in hopes of improving our services.
